How to use the tool
Follow these steps to generate a NICE-aligned care plan and download a PDF.
- Click Launch Tool on the homepage. It opens in a new tab. If it doesn’t, check your browser’s pop-up/tab settings.

Download the template sheet using the Download Template Sheet button in the tool, or
click here.
Do not include any patient-identifiable information.The Excel workbook has two tabs: Info and Patients. Please read the Quick Tips section on the Info tab, then complete as much relevant data as possible on the Patients tab.

Save your completed template as an Excel file (
.xlsx) with a simple identifier, e.g.Case001A.xlsx. - In the tool, use the file uploader to browse and select your completed template.
- Click Submit to upload your sheet.
- Click Run Management Plan to generate the care plan. You’ll see a loading spinner while it runs (typically ~15–30 seconds).
- Click Download PDF to save a PDF copy of the plan.
Important: AI can make errors. This tool provides draft clinical decision support and does not replace clinician judgement.
If you notice any errors or deviations from best practice, please consult a senior clinician (e.g., lead diabetic nurse, GPwSI, or specialist doctor).
